Transcripts

All transcript requests must be made in writing with the student's signature and the applicable charge paid before the transcript will be processed. No transcript will be issued if the student owes the College money or property. 

 

There are two types of transcripts as follows: 

  • Official: This transcript includes the Registrar's signature and the College Seal. This type is required by all college institutions and often purposes of employment. An official transcript can be mailed directly to the college/employer or given to the student. 
  • Unofficial: This transcript is similar in appearance to an Official transcript, but it does NOT include the Registrar's signature or the College Seal. Rather, the unofficial transcript is stamped, "UNOFFICIAL TRANSCRIPT ISSUED TO STUDENT". The student is free to make copies of an unofficial transcript, but it will not be recognized as an official document by any college or university.

How to Request a Transcript

 

You may submit a transcript request in two ways: 

 

1.) Mail/Fax-In - Please complete the Transcript Request Form . This form is unable to be submitted electronically. After careful review of the instructions given, you may mail or fax the form to the Registrar's Office. (You must have Adobe Acrobat Reader to view the form).

 

If you are unable to view or print the form, provide a written request to include:

  • Current full name and any previous names
  • Social security number
  • Birth date
  • Approximate dates of attendance
  • Home address and daytime or cell number
  • Number of copies request
  • Complete address of where the transcript is to be mailed
  • Student's signature
  • Payment (please see below for transcripts fees)

If you wish to pick up an official copy or have one mailed directly to your home, the cost is $8.00 (payment methods:  cash, VISA or Master Card, debit card or money order).  This copy will be official, but marked "TRANSCRIPT ISSUED TO STUDENT IN A SEALED ENVELOPE VOID IF SEAL IS BROKEN". Be advised that you should check with your intended recipient to see if they will accept a copy directly from you.

 

If you wish to have an official copy mailed to another school/university, the cost is $5.00 (payment methods:  cash, VISA or Master Card, debit card or money order).

 

If you need only an unofficial copy of a transcript, please indicate that on your request; there is no charge for an unofficial transcript.

 

*Note: If paying with a personal check, the request will be held for two weeks before processing.

2.) In-Person-Transcripts may also be obtained by visiting the Registrar's Office, located in Bertrand Hall on the ground floor, Room 6, during regular office hours 8:00 a.m.-5:00 p.m., Monday through Friday.

 

We try to process each request within two to three days of receiving the information.

If you are looking to request transcripts from Holy Rosary Academy, St. Catharine Academy, or Siena College (Memphis, TN) please follow the procedure below.

  • Student's name while attending
  • Year of graduation
  • Student's date of birth
  • Student's social security number
  • Address of location to send transcript information
  • $5.00 (cash or check) per transcript, which must be included at time of request
